Meals on Wheels Program Coordinator

Description:

  • Ensure adequate number of volunteers are available and scheduled for weekend meal delivery and ensure volunteer documentation is accurate
  • Add and remove clients to the routes according to site area using dept. software (Servtracker)
  • Maintain current and substitute volunteer list
  • Ensure sanitation of related equipment is performed on a daily basis
  • Work with volunteers on route delivery and entering meals into client track database
  • Inform case managers of client changes as observed by volunteer driver observations
  • Complete daily, weekly and monthly reports using dept. database (Servtracker)
  • As a substitute meal driver for delivery as needed following meal delivery timeframe (10 a.m. – 1:30 p.m.
  • Manage the rotating Saturday schedule and schedule other coordinator for frozen/grocery meal delivery at main office (NAM)
  • Order meals daily 3 days in advance with vendor
  • Participate in all department staff meetings and staff events
  • Organize distribution of special projects, i.e. Back to School, Christmas, Pet food, etc.

Education and Experience Requirements:

  • Associate’s degree in Social Services field
  • Previous experience working with seniors and volunteer management
  • Excellent organizational skills and proficiency in software programs
  • Must have valid driver’s license and good driving record

Bilingual Case Manager

Department: Family Violence Center

Responsible for:       

  • Facilitating client access to FVC services, NAM services and outside agencies.
  • Answering the FVC Hotline and perform telephone crisis counseling, crisis intervention, client assessment, and community referrals as appropriate.
  • Periodic coverage of the FVC after-hours on-call cell phone to provide telephone crisis counseling, crisis intervention, client assessment, and community referrals as appropriate.
  • Conducting telephone and face-to-face client intake interviews to determine client needs, client eligibility for services, and develop client safety plan.
  • Case management of assigned FVC client cases, including crisis interventions, assessments, and assisting clients to set appropriate goals or re-assess goals when needed.
  • Actively participate in on-going safety planning with FVC clients and Hotline callers.
  • Staffing all cases with Key Staff (DV Services Coordinator, SA Services Coordinator, Staff Therapist or FVC Director) including definition of problems, eligibility for services and identifying appropriate referrals.
  • Translation of brochures, educational materials and information about FVC services into Spanish, as needed.
  • Ensuring and protecting client confidentiality.
  • Operating within budgetary guidelines and in compliance with grant funding.
  • Develop working relationships internally and with external agencies.
  • Maintaining client files and all relative paperwork per FVC guidelines and policies.
  • Facilitating data entry into NAM’s database(s) and checking accuracy.
  • Assisting in the training of new staff, volunteers, and student interns.
  • Attending all NAM and FVC staff meetings, staff events, community events, and fund raising activities as scheduled.
  • Flexible work hours as needed to promote FVC’s mission.

Education and Experience Requirements:

  • Bachelor’s degree in Social Work or Bachelor’s degree in related field. (One year experience in a social service agency may be substituted for one year of college.)
  • Bilingual in Spanish and English required.
  • Must have dependable transportation.
  • Knowledge of domestic violence programs and professional social work approaches.
  • Experience in working with victims and volunteers.
  • Excellent organizational, verbal and written skills.
  • Computer skills (Word, Excel, and Microsoft Office) required.
  • Knowledge of other service agencies and their services.

Medical Assistant

  • Efficiently manage patient flow in area of responsibility.
  • Complete medical intake of patients including vital signs, height, weight, etc.

Bilingual Contract Therapist

Department: Family Violence Center

Responsible for:

  • Working with Spanish-speaking adult victims of family violence in individual and family counseling.
  • Working with child/adolescent victims of family violence in individual and group therapy.
  • Ability to define therapeutic approach for each client.
  • All intake assessments needed to determine a client’s eligibility for therapy services.
  • All record keeping, data collection, telephone calls and scheduling necessary for clients.
  • Storage, maintenance, ordering and inventory of all therapeutic supplies.
