Meals on Wheels Case Assessor

Position Summary:

The Case Assessor is responsible for assessing the clients for Meals on Wheels according to grantor requirements.  This position will conduct face to face home visits according to assessment schedule and complete community referrals as necessary according to client’s needs.

Description:                                                                                                     

  • Facilitate clients access to NAM services and outside agencies
  • Conduct initial intake by addressing client’s problems and needs, and determine eligibility for services
  • Conduct home visits and complete client initial assessments and annual reassessments maintain caseload and making referrals as applicable
  • Complete monthly grant report and submit in timely manner
  • Provide case management services related identified client needs to appropriate community resources
  • Address client changes as observed by volunteer driver observations
  • Perform outcome evaluations as applicable
  • Act as a substitute meal driver for meal delivery as needed following meal delivery time frame (10:00 am -1:30 pm)
  • Develop working relationship both internally and externally with appropriate agencies
  • Operate within the budgetary guidelines, ensure compliance with grant funding and provide reports to the proper authorities within the designated time frames.
  • Participate in all department staff meetings, staff events
  • Provide client information for special projects, i.e. Back to School, Christmas, Pet food, etc.

Supervisory Responsibilities: N/A 

Education and Experience Requirements:

  • Bachelor’s degree from four-year college or university in human services or related field; or one to two years related experience and/or training; or equivalent combination of education and experience.
  • Bi-lingual preferred, but not required

Computer Skills:     

  • “Microsoft Office Suite, especially Word and Excel- required
  • Database software-familiar
  • Internet Software- familiar

Special Requirements (Certificates, Licenses, Skills, Special Machines):

  • Prior experience working with seniors and volunteers required with excellent organizational skills and public speaking.

Position Summary:

Description: 

  • Greets donors, accepts donations and provides tax receipts.

THRIVE Specialist

Position Summary:

The THRIVE Specialist is responsible for providing career and educational counseling to clients who are unemployed or underemployed.  This position is responsible for program outcomes in scholarship awards and training completion.

Essential Duties and Responsibilities:

  • Meet all agency, program, contract and grant requirements, and all reporting schedules and budgets in an accurate and timely manner.
  • Conduct intake interviews and makes decisions regarding client eligibility and suitability for program according to United Way THRIVE grant and NAM policies.

Staff Therapist

Position Summary:

The Staff Therapist is responsible for providing therapeutic services to adult survivors of domestic violence and sexual assault.  The position will also supervise contract therapists who provide therapeutic services to children, adolescents, and Spanish-speaking clients. The Staff Therapist serves as the site supervisor for non-medical internships.  Position will be responsible for professionally interfacing with NAM staff, volunteers, clients, interns, community partners, as well as the FVC team.

Essential Duties and Responsibilities:

  • Working with adults/adolescents/child victims of family violence and sexual assault in individual, family, or group therapy
  • Supervision of contract therapists and therapist interns
  • Participate in case staffing/assessments meetings necessary to determine a client’s eligibility for therapy services
  • Availability to serve as the crisis intervention specialist, as needed, throughout NAM
  • Oversee the storage, maintenance, ordering, and inventory of all therapeutic supplies
  • Serve as site supervisor for social work, human service, and counseling student interns assigned to NAM
  • Develop working relationships with local institutions of higher learning to develop NAM’s internship program
  • Maintain regular communication with FVC Director regarding programming needs
  • Provide back-up support to the FVC Hotline to provide telephone crisis counseling, crisis intervention, client assessment, and community referrals, as appropriate

  • Produce statistical and narrative reports, as required by NAM, FVC or funders
  • Operating within budgetary guidelines and in compliance with grant funding

The FVC Staff Therapist is responsible for the direct supervision of contract therapists and intern therapists.  This responsibility includes, but it not limited, to FVC orientation, review of FVC standards and documentation, monthly file processing, and review therapeutic interventions.  Staff Therapist is also responsible for processing of contract therapy invoices and monitoring of related grant budgets.

Education and Experience Requirements:

  • LCSW licensing is requirement, plus four to ten years related experience and/or training. Current State of Texas licensure is a requirement.
  • Prior experience working with victims/survivors of domestic violence and/or sexual assault is required.
