Grant Funded Coordinated Access Assessor/Navigator

The Assessor/Navigator performs a variety of routine and complex technical field and office work in the area of homeless housing population.  The Assessor/Navigator will schedule appointments for assessment housing opportunities for homeless families/chronic individuals as they meet HUD guidelines for homelessness.  The Assessor/Navigator will outreach individuals/families at various community locations to provide information and conduct screenings to determine eligibility for the program.  The Assessor/Navigator will work with eligible individuals to identify barriers to obtaining housing and to collaboratively develop an individualized housing plan.  The Assessor/Navigator will support each individual’s efforts to resolve barriers to obtaining housing. This position is grant funded from July 1, 2017 through June 30, 2018.

Essential Duties and Responsibilities:

  • Screen homeless persons for housing eligibility using the Coordinated Access assessment tool.
  • Provide client referral to the appropriate housing intervention and potential housing sites.
  • Pull housing referral from HMIS and begins the next step process to enrollment.
  • Assist individuals that are homeless in gathering all documents necessary to complete a housing program enrollment.
  • Accompany clients to all housing appointments and serve as clients’ primary advocate during the housing search and selection process and prior to transfer to the housing case manager.
  • Transfer all required homeless and housing documentation to the housing case manager.
  • Ensure all data is collected and entered accurately and in a timely manner into HMIS.
  • Be available to conduct assessments at different Assessment Hubs located throughout the Continuum of Care.
  • Attend all Housing Assessor, Navigator, and HMIS trainings in order to fully utilize the Coordinated Access system accurately.
  • Provide high quality services to those accessing the homeless services system.
  • Provide regular feed-back to management level staff of collaborating agencies regarding the front-line Coordinated Access process.
  • Provide feed-back to Coordinated Access Workgroup regarding Policies and Procedures of the Coordinated Access system.
  • Participate in case conferences as needed.
  • Participate in Coordinated Access Transition Team meetings as needed.
  • Work some weekends or evenings as required.
  • Attend NAM staff meeting, staff events and fundraising activities as scheduled

Other duties may be assigned.

Education and Experience Requirements:

  • Bachelor’s degree from four-year College or University; or one to two years related experience and/or training; or equivalent combination of education and experience.

  • “Microsoft Office”, special Word and Excel – required
  • Must have a valid Texas Driver’s License, good driving record and able to use personal vehicle for reimbursable work related activities.

Bilingual Case Manager

The Bilingual Case Manager is responsible for providing support services to domestic violence victims/survivors in the client’s preferred language of English or Spanish.  Case management services will be provided through a client-centric model with a specific focus on client needs, community resources, and safety planning.  Position will be responsible for client services, professionally interfacing with clients and community partners, and working as part of the FVC client services team.

Essential Duties and Responsibilities:

  • Facilitating client access to FVC services, NAM services and outside agencies
  • Answer the FVC Hotline to provide telephone crisis counseling, crisis intervention, client assessment, and community referrals, as appropriate
  • Periodic coverage of the FVC after-hours, on-call cell phone to provide telephone crisis counseling, crisis intervention, client assessment, and community referrals, as appropriate
  • Conducting telephone and face-to-face client intake interviews to determine client needs, client eligibility for services, and develop client safety plan
  • Case management of assigned FVC client cases, including crisis interventions, assessments, and assisting clients to set appropriate goals or re-assess goals as needed
  • Actively participate in on-going safety planning with FVC clients and Hotline callers
  • Staffing all cases with Key Staff (DV Services Coordinator, SA Services Coordinator, Staff Therapist or FVC Director) including definition of problems, eligibility for services, and identifying appropriate referrals
  • Translation of brochures, educational materials and information about FVC services into Spanish, as needed
  • Ensuring and protecting client confidentiality
  • Operating within budgetary guidelines and in compliance with grant funding
  • Develop working relationships internally and with external agencies
  • Maintaining client files and all relative paperwork per FVC guidelines and policies
  • Facilitating data entry in NAM’s database(s) and checking accuracy
  • Assisting in the training of new staff, volunteers, and student interns
  • Attending NAM and FVC staff meetings, staff events, community events, and fundraising activities as scheduled
  • Flexible work hours as needed to promote FVC’s mission.

Education and Experience Requirements:

  • Bachelor’s degree from a four-year college or university and a minimum of three years’ experience working in a social service agency or equivalent combination of education and experience.

Language skills:     

  • Proficiency in English and Spanish are required for this position. Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.  Ability to write routine correspondence, reports, business correspondence and procedure manuals.  Ability to speak effectively before groups of customers or employees of organization.

Part-Time Data Entry/Volunteer Clerk

Bilingual Contract Therapist

Department: Family Violence Center

Responsible for:

  • Working with Spanish-speaking adult victims of family violence in individual and family counseling.
  • Working with child/adolescent victims of family violence in individual and group therapy.
  • Ability to define therapeutic approach for each client.
  • All intake assessments needed to determine a client’s eligibility for therapy services.
  • All record keeping, data collection, telephone calls and scheduling necessary for clients.
  • Storage, maintenance, ordering and inventory of all therapeutic supplies.
  • Maintaining regular communications with FVC Director and Staff Therapist regarding scheduling and programming needs.
  • Actively participate in a monthly processing meeting with other contract therapists, led by the FVC Staff Therapist.

Education and Experience Requirements:

  • Master’s level degree and licensure in counseling field. (candidates that have completed clinical hours and prepared for exam will be considered)
  • Bilingual (English/Spanish) required.
  • Experience in working with adult and child/adolescent victims of domestic violence.
  • Skilled in working with play, sand and art therapy materials.
  • Skilled in utilizing appropriate treatment models for targeted populations.
  • Excellent organizational skills and attention to record keeping.
